HeartWize
A Specialized Treatment Program for Patients with Heart Disease
Center for Hospice Care’s HeartWize program offers patients the option of managing their own care and the ability to optimize their quality of life. It is designed for patients with maximally treated heart disease who would like to avoid further rehospitalizations and are not candidates for further surgical procedures. HeartWize can assist patients, their loved ones and their physicians manage the symptoms of heart disease at home.
HeartWize Goals
- Increase the patient’s ability to copy with their illness
- Increase the caregiver’s confidence in their ability to cope
- Closely monitor and manage symptoms to reduce crisis episodes
- Reduce hospital stays and emergency room visits
- Improve the patient’s quality of life and help them accomplish their goals
HeartWize Accomplishes This Through
- Regularly scheduled visits by CHC staff who have specialized training in cardiac disease evaluation and symptom management
- A cardiac medication kit placed in the patient’s home
- Program oversight by our hospice medical director
- Program consultation with area cardiologist
- Education and treatment materials specifically designed for end-stage cardiac patients
